Specialized Care

Everyone develops different needs as they get older. Five levels of care have been created at Hearth & Home at Van Wert so that each resident can have care customized to suit their needs.

Level One:

• Emergency call system includes a personal pendent worn by the resident for immediate response from the staff
• Fresh, home-style cooking, incorporating personal preferences and favorite recipes. All meals and snacks included
• Full use of the family-style home, only a few steps from dining and living areas
• Social life options geared to the individual interests of current residents
• Housekeeping & laundry services provided. Personal laundry done individually in non-commercial washers
• Nurse on call 24 hours plus excellent staffing patterns that insure one-on-one care for each resident.
• Reminders for medication (Medications will be billed separately)
• Partial assistance with bathing, dressing and grooming
• All utilities, including cable TV included. Personal phone service must be made through a local company.

Level Two:

• Includes Level One services plus full assistance with bathing, dressing, walking and medication.

Level Three

• Includes Level One and Two services plus assistance with eating, transferring and occasional incontinence

Level Four

• Includes Levels One, Two and Three services plus regular daily incontinence, feeding and ambulation assistance. Attention given to some short-term memory loss with cueing and redirecting.
